Child Consent Form
A child consent form is designed to protect your child in case of an emergency when parents are not present. This form will help to avoid a possible delay in the treatment of an ill or injured child who is in someone else’s care, such as a babysitter or grandparent. You might also want to complete a child consent form when traveling with friends or under other similar circumstances.
Simply enter the information on the form linked below, then print it out, sign it, and give it to the people who will be responsible for your minor child(ren). If care or treatment is needed, the parties granted consent can take the document with them to the hospital or doctor.
Frequently Asked Questions
Q: What Is a Pediatrician?
A: A pediatrician is a medical doctor who specializes in the care of children, from birth through age 18.
Q: What Do Pediatricians Do?
A: Pediatricians provide comprehensive health care for children from routine preventative care for those in good health to the diagnosis and treatment of acute and chronic illnesses. In addition to medical care, pediatricians offer guidance and support to parents on important topics such as growth and development, nutrition, safety, and emotional well-being. Their goal is to promote lifelong health and wellness for each child.
Q: How Often Should My Child See the Pediatrician?
A: Regular pediatric visits are essential to your child’s health, starting in infancy. These well-child visits allow the doctor to monitor your child’s growth, development, and overall health. Your pediatrician will provide a recommended schedule, with visits occurring more frequently during the first two years of life and gradually becoming less frequent as your child grows, particularly after age 2 and 3.
Q: What Can I Expect at My Child’s Checkup?
A: Routine visits allow the pediatrician to observe the progress of your child’s physical and mental growth and development, including, but not limited to, the following:
- Height
- Weight
- Growth and Developmental Screenings
- Immunization Updates
- Anticipatory Guidance and Milestone Developments
Q: How Do I Choose a Pediatrician?
A: Before you make a final decision on a pediatrician, it can be helpful to schedule an introductory visit to get to know the provider and their team. Ask some of the following questions to get to know your potential provider and begin developing a relationship.
- What Is Their Childcare Philosophy?
You will want to ensure your thoughts around everything from breastfeeding to vaccinations to sleep training align with the doctor you choose.
- How Are Emergencies Handled?
Are same-day appointments for emergencies accommodated? What about after-hours emergencies?
- What Hospital Are They Affiliated With?
This will be important when it comes to giving birth and if emergencies arise with your child.
- Do They Have Any Subspecialties?
For example, do they have additional qualifications in pediatric critical care, pediatric gastroenterology, or pediatric cardiology?
Q: What Do I Need to Bring to My First Visit?
A: For new patients, please bring your child’s previous medical records, including immunization history, health insurance information, completed new patient forms, and pharmacy contact information. We also encourage you to bring a list of questions or concerns, so we can ensure all of your child’s health needs are thoroughly addressed.
